Our team of caring, compassionate medical staff is dedicated to listening and acting as guides along your path to maintaining wellness. Your PCP is your resource for staying healthy. Your PCP is also the first person you should contact if you have a health concern.

Locations: Keene, Walpole, and Winchester, New Hampshire

Our PCPs provide most of your health care needs, including prevention and wellness, acute care, and chronic care. From yearly checkups to coordinating care for an injury or disease, we are your medical home team. Dartmouth Hitchcock Medical Center (DHMC)

Location: Lebanon, New Hampshire

Our primary care departments provide accessible, comprehensive, and compassionate care for you and your family. There may be wait lists for new patients who want to sign up with a primary care provider at our General Internal Medicine practice. Our team is working hard to address staffing shortages. Mt. Ascutney Hospital and Health Center (MAHHC)

Locations: Windsor and Woodstock, Vermont

MAHHC is committed to providing you with the best possible care in all situations. Southwestern Vermont Medical Center (SVMC)

Locations: Bennington, Northshire, Deerfield Valley, and Pownal, Vermont; and Hoosick Falls, New York

Primary Care services include family medicine clinicians, who specialize in the care of the whole family from children through adulthood; internal medicine clinicians, who specialize in treating adults with more complicated conditions; and pediatricians, who specialize in treating the illnesses of childhood. Some practices of Southwestern Vermont Medical Center are accepting new patients.
